Care for Older People
At Kent SCP, we provide a range of services for the elderly, from simple practical support with everyday tasks, like housework, shopping and laundry, to personal care, meal preparation and nursing support. In many cases, our staff also help client to maintain family and social lives, often escorting them on trips outside the home. We also specialise in supporting people with particular age-related health conditions like dementia and Parkinson’s disease.
Kent SCP’s services are bespoke to each client. Each care plan is individually designed in partnership with the client depending on their specific needs. This person-centred approach gives our elderly clients more choice and control over when and how their support is provided.
We take care to match our care assistant with specific clients to ensure suitability, both in terms of skill and personality.
Physical & Learning Disabilities
We always aim to support our physically-disabled clients to lead active social lives, assisting them outside as well as inside the home. We support both adults and children who’ve disabilities of all kinds and, with specially-trained, fully-vetted staff, we help our younger clients have the best start to life they can. We believe children’s lives should be fun-filled and active, no matter their disability.
Even if disabled, receiving care shouldn’t mean not being able to live life to the fullest. We encourage our clients to be as independent as possible, offering them as many opportunities to make choices and decisions that affect their own lives, whether they’re living at home, with family or in residential homes.
Employing a person-centred approach, we will consult with a client’s family, friends and medical professionals as to the best way forward. When a client does not bear the mental capacity to make decisions on their own, we always look to do so in their best interests.
In cases where we’re faced with challenging behaviour, our care assistants receive extensive training to help manage those situations while still maintaining a client’s dignity, respect and safety. Importantly, we take special care to ensure that we match the right care assistant with the right client, looking to build a relationship centred on trust and companionship.
Live-in & Extra Care
Live-in care is a great option for those who require high levels of support but who don’t want to sacrifice their independence or leave the home they know and love. Choosing live-in care gives the client complete control over their own schedule, such as determining meal times, and creates an opportunity to forge a real bond with their care worker. Our live-in care assistants are all trained to provide quality support across an array of practical activities from personal care, meal preparation and running errands through to managing correspondence. They are also given specialist training to accommodate your particular health needs.

We take the time to find a carer who is best suited to your needs and lifestyle; our objective is to pair you with someone with whom you can build a real, lasting relationship.
Extra Care, sometimes known as assisted or supported living, provides clients with access to high levels of care without having to sacrifice all of their autonomy. Our schemes operate in purpose-built properties, which provide accessible and safe housing for older people who are unable to live completely independently. Our skilled and specially trained care teams are on site twenty-four hours a day, ready to provide support as and when it is required. Our Extra Care Schemes strike an important balance for service users who require high levels of support but don’t wish to yield all of their independence.
Whilst our Extra Care Schemes have a communal spirit, the care we give our clients is highly personalised. We build tailored care plans which accommodate not just the assessed needs of the client, but also their unique preferences and aspirations. This enables us to deliver a personalised, outcome-focused service. So, whilst tenants in our Extra Care schemes can access the benefits of their community, they can still count on getting the person-centred care they desire.
The cares at our Extra Care sites across Kent are passionate about creating vibrant communities in which service users can play as active a role as they choose. In addition to developing a calendar of engaging social activities, we also run scheme-wide initiatives which help service users to stay fit and healthy, such as onsite GP clinics, nutrition workshops and exercise programmes.
Dementia Care
Kent SCP’s Dementia Care teams receive specialist training to equip them with deep knowledge and understanding of the disease in its various forms and stages. This knowledge is coupled with ongoing skills training to ensure that our clients have access to the cutting-edge techniques which will help them to live safe and active lives. Our dementia support plans are always based on the needs of the individual service user, putting the person, not the disease, first.
We provide a range of practical support to our dementia care clients; we assist with everyday activities such as personal care and dressing through to meal preparation as well as help administering medication. In addition to this hands on support we also offer much needed companionship, providing the sort of social interaction which can help dementia patients to stay engaged and active.
We understand that this disease effects not only the person suffering from dementia, but also their families. We also recognise that welcoming someone into your home to care for a vulnerable loved one requires a great deal of trust. That is why we work closely with the families of our clients to develop tailored care plans and provide regular opportunities for feedback from both parties.
Reablement
Returning home after a spell in hospital can be daunting for anyone, especially where they have lost physical strength or confidence in their own abilities, sometimes in relation to the simplest of tasks. Our expert reablement teams comprise specially trained support staff who work in partnership with occupational therapists, physios, nurses and a range of other professionals to work with the individual in achieving a smooth transition to partial or full independence.
Unlike some other approaches to care support, reablement places a particular emphasis on supporting the individual to help themselves. Short and long-term goal-setting allows the service user to make small steps towards independence. Supported by our experienced and highly-trained staff, the individual will, day by day, stretch the limits of their capability in line with agreed goals, at each step regaining skills and confidence. Progress is closely monitored and the reablement plan adjusted as necessary to respond to the service user’s changing abilities.

End of Life Care
Working closely with palliative nurses and other care teams, our end of life teams help clients to plan and manage the final weeks, days and hours of life, managing pain and discomfort and putting into effect agreed plans of care. Our staff are trained and supported to understand and cope with the unique challenges and pressures of this type of care, providing unobtrusive, sensitive support which promotes dignity.
Receiving end of life care at home, rather than in a hospital or hospice, allows the service user to be close to their family and to draw comfort from familiar surroundings. Family members can also be more confident that their loved one is receiving a personalised programme of care, helping to raise the quality of the time they have left.
